These SMS Terms & Conditions (“SMS Terms”) govern text messages sent by or on behalf of Grand Hospice, Inc. (“Grand Hospice,” “we,” “us,” or “our”). By opting in to receive SMS/text messages from Grand Hospice through a website form or other authorized method, you agree to these SMS Terms.
1. SMS Program Description
By opting in to SMS communications from Grand Hospice, you agree to receive text messages related to your interactions with Grand Hospice.

Current Grand Hospice patients and caregivers should use the clinical contact information provided by their hospice care team for urgent patient-care needs.

By voluntarily providing your mobile telephone number and opting in to SMS communications, you consent to receive SMS/text messages from Grand Hospice at the number provided.
Consent to receive SMS messages is not a condition of receiving hospice care, healthcare services, or other services from Grand Hospice.
Your consent applies only to the communications for which you have enrolled or otherwise provided consent.
3. Message Frequency
Messaging frequency may vary.
The number and frequency of messages you receive will depend on your interactions with Grand Hospice, the services you have requested, and the nature of our communications with you.
4. Message and Data Rates
Message and data rates may apply.
Your wireless carrier's standard messaging and data charges may apply to messages sent or received in connection with the Grand Hospice SMS program. Grand Hospice is not responsible for charges imposed by your wireless carrier.
5. Opting Out
To opt out at any time, text STOP.
You may reply STOP to any Grand Hospice SMS message to discontinue SMS communications. After submitting an opt-out request, you may receive a final message confirming that you have been unsubscribed.
Opting out of SMS communications does not prevent Grand Hospice from communicating with you through other methods when permitted or required by law, including communications necessary for the provision or coordination of healthcare services.
