A CLEARER NEXT STEP
A conversation. An evaluation.
A plan that starts with you.
You can call with questions before you know what comes next. We will listen to your concerns and explain the evaluation process in plain language.
01 — Tell us what matters
Share what prompted your call, your questions, and the patient’s goals. We will help clarify whether a hospice evaluation may be appropriate.
02 — Understand the options
An evaluation helps the clinical team understand the patient’s needs and eligibility. Ask about services, coverage, caregiving, and how care would fit into daily life.
03 — Plan together
If hospice is appropriate and chosen, we work with the patient, family, physician, and care team to develop an individualized plan of care.

What is hospice care?
Hospice provides comfort-focused care for people living with a terminal illness. Our team helps manage pain and other symptoms while supporting each patient’s emotional, social, and spiritual needs. We also provide guidance and support for family caregivers.
Who may be eligible?
Hospice may be appropriate when a physician believes a person could have six months or less to live if the illness follows its expected course. Eligibility is based on the patient’s overall condition, and our team can provide an evaluation to help determine whether hospice care is appropriate.
Where is care provided?
Hospice care is provided wherever the patient calls home, including a private residence, assisted living community, or nursing facility. When a different level of care is needed for symptom management or caregiver relief, our team helps coordinate those arrangements.
Does care stop after six months?
No. Hospice care may continue beyond six months as long as the patient remains eligible and the required recertifications are completed.
What does insurance cover?
Medicare Part A, Medicaid, and most insurance plans cover hospice services for eligible patients. Some copayments may apply, depending on your coverage. Routine room and board is generally not covered. Our team will explain your benefits and any potential costs before care begins.
How are caregivers supported?
Caregivers receive education, guidance, and ongoing support from the hospice team. Depending on the patient’s needs, the plan of care may include symptom management, medications, medical equipment and supplies, counseling, and short-term respite care. Our team is also available around the clock when questions or concerns arise. Medicare’s hospice benefit includes inpatient respite care for up to five consecutive days when eligibility requirements are met.
